Support Coordination

What is Support Coordination?

Support Coordination is a service under the NDIS that helps participants understand, navigate, and implement their NDIS plans effectively. A Support Coordinator works with you to connect with providers, services, and community resources that align with your goals, ensuring you get the most out of your plan. They help build your capacity to manage supports independently over time.

 

Journe Support Coordinators

Journe has a strong team of support coordinators who specialize in Mental Health and Psychosocial supports in the Hunter region. 

 

We come from many different backgrounds and can support NDIS participants when facing a range of challenges and transitions, such as: 

 

  • Transitioning from hospital 

  • Young people living in Aged Care Facilities

  • Transitioning from custody

  • Living with addiction

  • Living with comorbidities

  • Under Guardianship 

  • Experiencing Homelessness

Types of Support Coordination

1. Support Coordination

This level of support is for participants who need help understanding their plan, choosing providers, and coordinating their supports. A Support Coordinator will:

  • Assist in finding and connecting with services.

  • Help manage service agreements and bookings.

  • Support you in building your confidence to manage your plan over time.

2. Specialist Support Coordination

For participants with more complex needs, Specialist Support Coordination provides a higher level of support. A Specialist Support Coordinator:

  • Works with participants who face significant barriers in accessing services.

  • Helps resolve complex situations that impact service delivery.

  • Collaborates with multiple providers and systems (e.g., health, justice, housing) to ensure seamless support.

3. Psychosocial Recovery Coaching

Designed for people with psychosocial disabilities (e.g., mental health challenges), a Psychosocial Recovery Coach:

  • Focuses on recovery-oriented support.

  • Provides coaching to help build skills and resilience.

  • Works collaboratively with clinical and community services to support long-term recovery.

Aleisha Reasbeck

Aleisha is a compassionate, determined and resourceful person who is rewarded by being a part of a participant’s journey to lead a meaningful life. With a lived experience and a son with a psychosocial disability, Aleisha understands the challenges and sometimes the stigma that can go with a diagnosis. Being a good listener and having a strong legal background she is able to support participants to navigate the complexities of the NDIS.

Jade Doley 

Jade brings clarity and spirit to support coordination. She has an extensive background in adult and youth mental health, and has been specialising in mental health support coordination for six years.
Jade draws upon the skills she has obtained through working in both the Housing and Accommodation Support
Initiative and Partners in Recovery programs. Jade has specialist knowledge in trauma and complex mental health and is passionate about working with youth and young adults. She also has a significant background in yoga, which is evident in her delivery of support coordination.

Samantha Kelly

Samantha comes to Journe with 10 years of experience in mental health and community sector management. She has been specialising in mental health support coordination. Samantha draws upon the skills she has obtained through working in Housing and Accommodation Support Initiative (HASI) and Partners in Recovery (PIR). Samantha also brings with her an extensive specialist knowledge regarding psychosocial disability. Samantha is compassionate and committed to supporting participants on their recovery journeys to identify and achieve their own unique goals.

Sharon Kennedy

With 13 years in disability employment, Sharon has walked beside hundreds of people as they discovered purpose, built confidence, and
redefined what’s possible. Believing that small steps make big dreams, she helps people find the quiet wins, the next step, or the breakthrough moment.

Sharon brings maturity, insight, and a genuine commitment to
empowering others. Whether it’s navigating services, coordinating care, or forging a path to employment, she is practical, responsive, and
deeply committed to helping people move forward with clarity and confidence.

Bianca Roberts

Bianca draws on lived experience, professional knowledge, and a heartfelt commitment to her work. With five years in the NDIS space, she has naturally progressed into Support Coordination and Recovery Coaching, where her strengths truly shine. She specialises in supporting young adults aged 18 to 25, particularly those navigating gender identity, LGBTQ+ experiences, anxiety, and depression. Her compassionate approach is grounded in a deep personal understanding and a genuine belief in recovery.

With a strong passion for helping others actively shape their journey, Bianca is driven by the opportunity to make a meaningful difference. She finds purpose in empowering people to take confident steps toward their goals and is committed to walking beside them with energy, authenticity, and care.
