Our services support people with a psychosocial disability to build their capacity in a community setting. Through these services, our aim is to empower people to be comfortable with who they are and to live their life in a way that is meaningful to them.

Support Coordination

What is Support Coordination?

Support Coordination is a service under the NDIS that helps participants understand, navigate, and implement their NDIS plans effectively. A Support Coordinator works with you to connect with providers, services, and community resources that align with your goals, ensuring you get the most out of your plan. They help build your capacity to manage supports independently over time.

 

Journe Support Coordinators

Journe has a strong team of support coordinators who specialize in Mental Health and Psychosocial supports in the Hunter region. 

 

We come from many different backgrounds and can support NDIS participants when facing a range of challenges and transitions, such as: 

 

  • Transitioning from hospital 

  • Young people living in Aged Care Facilities

  • Transitioning from custody

  • Living with addiction

  • Living with comorbidities

  • Under Guardianship 

  • Experiencing Homelessness

Types of Support Coordination

1. Support Coordination

This level of support is for participants who need help understanding their plan, choosing providers, and coordinating their supports. A Support Coordinator will:

  • Assist in finding and connecting with services.

  • Help manage service agreements and bookings.

  • Support you in building your confidence to manage your plan over time.

2. Specialist Support Coordination

For participants with more complex needs, Specialist Support Coordination provides a higher level of support. A Specialist Support Coordinator:

  • Works with participants who face significant barriers in accessing services.

  • Helps resolve complex situations that impact service delivery.

  • Collaborates with multiple providers and systems (e.g., health, justice, housing) to ensure seamless support.

3. Psychosocial Recovery Coaching

Designed for people with psychosocial disabilities (e.g., mental health challenges), a Psychosocial Recovery Coach:

  • Focuses on recovery-oriented support.

  • Provides coaching to help build skills and resilience.

  • Works collaboratively with clinical and community services to support long-term recovery.
